The slope-intercept form of an equation is y = mx + b.

m = Δy/Δx
b = the point at which the line intersects the y axis (where x=0).
y = m(0) + b gives us the the point (0,b) on the line.

From a graph determine the slope of the line (rise over run) and the point and which the line intersects the graph, then fill in the values for m & b.

For example, you notice the slope is 2/1, and intersects y at (0,5).  The equation is:  y = 2x + 5
