What are some of the methodological challenges of adapting a measurement instrument for use in different languages and in different cultural environments?

I believe the challenge is to create standard instruments that would fit across the languages and cultural environments. Given the fact that people are born with different styles of understanding concepts, to finding the common ground for all that would not somehow get too distant from the expected responses.
