slope = (y2-y1)/(x2-x1)

= (1-6)/(2-1)

= -5/1

=-5

point slope form

y-y1=m(x-x1)

y-6 = -5(x-1)

y-6 = -5x +5

add 6 to each side

y = -5x+11

this is slope intercept form
