why might a crowded waiting room be a sign of inefficiency rather than the providers popularity

The provider is unable to be efficient enough to deal with all of the customers

A crowded waiting room could mean both, but it can also mean it is inefficient. Backed up waiting rooms can mean a short staff, no stock, or a plethora of other things, let alone their inefficiency to deal with the customers.
